Nomination Checklist

Submit the following materials to the address provided within the nomination form. Deadline for receipt of nominations for the 2010 competition will be announced shortly.

  • Nomination statement/letter signed by the nominator;
  • A brief resume (250 words) to inform jurors about the Nominator's background and assist them in assessing the credibility of the application;
  • Two letters of support from seconders, with original signatures;
  • Six copies of all support documents;
  • Six copies of books, CDs, video or sound recordings, media footage, catalogues, or other primary representation of the work, as applicable;
  • Six copies of the signed nomination form (the original and five copies);
  • Six copies of the biography of the principal creator or creators of the nominated Masterwork;
  • Include a self-addressed stamped envelope if you would like any of the support materials returned.
